【摘要】 目的:观察自行设计的上胸椎前路钛板内固定系统的固定效果。方法:16只成年波尔山羊,随机分成4组,即自行研制的上胸椎前路钢板固定组(A组),前路未塑形Orion钢板固定组(B组),前路塑形Orion钢板固定组(C组),单纯植骨组(D组),对A、B、C组行T3椎体次全切除+髂骨植骨+内固定术,D组行T3椎体次全切除+髂骨植骨术。于术后3个月行影像学和组织学检查,观察内固定和植骨融合效果及相关并发症。结果:A、B、C组山羊均存活,D组山羊因麻醉意外死亡1只。存活山羊均未出现与内固定相关的并发症。术后3个月影像学融合效果:A组均Ⅰ级融合;B组Ⅰ级融合1只,Ⅱ级融合3只;C组Ⅰ级融合2只,Ⅱ级融合2只;D组Ⅱ级融合2只,Ⅲ级融合1只。组织学融合效果:A组Ⅰ级融合3只,Ⅱ级融合1只;B组Ⅰ级融合2只,Ⅱ级融合2只,Ⅲ级融合1只;C组Ⅰ级融合1只,Ⅱ级融合2只,Ⅲ级融合1只;D组Ⅲ级融合2只,Ⅳ级融合1只。A组优于B、C、D组(P<0.05),B、C组优于D组(P<0.05),B组与C组之间无明显差别(P>0.05)。结论:上胸椎前路钛板内固定系统在山羊胸椎次全切除后行植骨融合内固定安全、可靠,融合效果好。 |

【Abstract】 Objectives: To evaluate the fusion effect and stability of self-designed upper thoracic vertebra instrument in goat. Methods: 16 adult Boer goats were divided into four groups randomly: group A underwent self-designed upper thoracic vertebra anterior titanium plate, group B with non-shaped Orion plate, group C with shaped Orion plate, and group D included normal thoracic vertebra specimens. The goats of group A, B and C underwent T3 corpectomy and bone graft, while group D underwent T3 corpectomy alone. The fusion effects and complications were observed by radiological and histological examinations 3 months later after the surgery. Results: One goat in group D died of anesthesia accident, while the others survived with no instrument-related complication. Imaging fusions were as follows: grade Ⅰ in 4 goats of group A, 1 of group B, 2 of group C; grade Ⅱ in 3 goats of group B, 2 of group C and D; grade Ⅲ in 1 of group D. Histological fusions were as follows: grade Ⅰ in 3 of group A, 2 of group B, 1 of group C; grade Ⅱ in 1 of group A, 2 of group B and C; grade Ⅲ in 1 of group C, 2 of group D; grade Ⅳ in 1 of group D. Group A was better than the other 3 groups(P<0.05), and group B and C were better than D(P<0.05). No significant difference between B and C was found(P>0.05). Conclusions: The anterior titanium plate after upper thoracic vertebra corpectomy proves to have satisfactory reliability and better fusion effect in goat. |
