  目的：探讨腰椎管狭窄症的手术要点及椎弓根内固定在腰椎管减压手术中的作用和意义。方法：总结1999年1月～2001年6月采用广泛椎板减压、椎弓根内固定及椎间关节融合、横突间植骨治疗的52例腰椎管狭窄症病例的临床资料。结果：52例随访6个月～30个月，平均14．5个月，术后跛行改善者52例(100％)，下肢肌力障碍恢复36例(69．2％)，肌萎缩16例均有所恢复；36例感觉障碍者，完全恢复10例(27．8％)．部分恢复20例(55．66％)，无明显恢复6例(16．7％)，4例术前没有皮肤感觉障碍的病人术后出现小腿或足背外侧皮肤麻木，3个月后恢复。结论：在腰椎管狭窄症减压手术中，椎弓根内固定对有结构性和动力性腰椎滑脱的病人可起到使滑脱椎体复位、相邻椎体间稳定及椎间融合的作用；并可使摘除椎间盘后的相关间隙高度得到维持，减少残余或进一步变性的组织向椎管内脱出。
